How do I request for a referral control sheet (RCS)? 

You can request RCS in two (2) ways: 

 

Via email request 

 

1. Use email subject “RCS-REQUEST" then your account number 
e.g., RCS-REQUEST 00-00-00000-00000-00 

2. Attach the following files: 

    - Standard Excel Template (attached in this link) 

     - 1 Government-issued ID 

     - Copy of your AVEGA ID 

3. Email request to [email protected] 

 

Note that the request form should be completed based on instructions, and requirements should be submitted to avoid delays in processing. 

 

Via Agora App 

 

RCS1- Consultation 

  1. Log in to the Agora app and click “Request for eConsultation Form”
  2. Select “Continue request for eConsultation. 
  3. Fill out and submit form 
  4. Receive eRCS1 via registered email address 
  5. To view transaction request, click “Transaction History” 

Note: 

  • If your chief complaint is not showing, kindly type and select “General Check-up”. 
  • Please bring two (2) printed copies of your eRCS prior to your consultation. 

 

 

RCS2- Diagnostic 

  1. Log in to the Agora app and click “Request for eDiagnostic Procedures Form” 
  2. Select “Create request now” 
  3. Fill out and submit form
  4. Receive eRCS2 via registered email address 
  5.  To view transaction request, click “Transaction History” 

Note: 

  • If you can’t find your procedure, call Avega hotline or send an email to [email protected] or [email protected]. 
  • Please bring two (2) printed copies of your eRCS prior to your procedure. 

 

 
The validity of eRCS1 and eRCS2 is three (3) days. Cancellation can be done through the Agora app.
