New Dependents' Enrollment Process: Step-by-Step

Step 1: Log in at www.benefitsmadebetter.com. 
Note: You need to select flexi-medical allowance option to proceed.

Step 2: Click “Dependent’s Enrollment” tab, then select “Proceed to Dependents Enrollment” to start the enrollment process. 

Note: If you select "Option 1" you will initially be redirected to “Proceed to Dependents Enrollment” page.

Step 3: Verify/update personal information. Remember to save any changes before moving to Step 4.

Step 4: Identify the dependents you want to enroll.

Step 5: Fill out Dependent’s Form.  

Important: Do you want to skip a dependent? You still need to provide their information and reason for skipping.

Step 6: Select plan option (as applicable) then click “Continue”.

Step 7: For final confirmation, verify your dependent/s’ details. Then click “I hereby certify that the above details are true and correct”.

Step 8: Click “View DECF and Data Privacy Consent Forms.”

Step 9: Read the Dependents Enrollment Confirmation Form and select “Download DECF and Data Privacy Consent”.

Step 10: Confirm your acceptance through Two-Factor Authentication (2FA).

To accept, you must enter the confirmation code that will be sent to your registered mobile number or email address. 

  • If you exceed 3 attempts at SMS confirmation code delivery, the code will be emailed to you. 
  • If you do not confirm using either SMS or email, you will receive a text-to-speech call with the code. 
  • If you do not receive a code, please wait for 5-15 minutes before trying again. 

Remember that completing Two-Factor Authentication (2FA) automatically confirms your dependents’ endorsement to the HMO plan. Your dependents’ enrollment cannot be cancelled until September 30, 2024 unless they meet removal criteria. 

 

Do you have dependents aged 18 and above? Submit their signed Data Privacy Consent Form. 

 

 

Step 11: Check your email for your transaction details and the list of documents to be uploaded. 

 

Step 12: Go to “Required Documents” and upload supporting documents.

Here is the list of required documents per dependent:

  • Spouse - Marriage certificate
  • Child- Birth certificate
  • Parent - Marriage certificate
  • If unmarried - Principal member’s birth certificate
  • If the dependent’s surname is different from the principal – Principal member’s birth certificate
  • Siblings- Birth certificate
