What are the documents I need to upload to support the enrollment of my dependents?

Below is the list of documents you need to upload to support the enrollment of your dependent:

  • For existing dependents: If you enrolled a dependent above 18 years old, you need to submit the Data Privacy Consent Forms with signature of your legal-age dependents. No need to submit other supporting documents for existing dependents.
  • For Newly Enrolled Dependents: You need to upload the following:
    • Husband/Wife - Marriage Certificate
    • Child/Children - Birth Certificate
    • Parent/s - Marriage Certificate 
    • Siblings - Birth Certificate
    • If dependent’s surname differs from the Partner’s – Partner’s Birth Certificate
    • If you enrolled dependent 18 years old and above - Data Privacy Consent Forms with signature of your legal-age dependents
    • Domestic partner/Same-gender partner:
      • Joint affidavit of domestic partnership certifying that all the criteria are met 
      • Documentation showing proof of financial interdependence, such as shared lease, joint bank account or ownership of properties 
      • Barangay certification of co-habitation stating that the partner and domestic partner live in the same address including duration of their residence therein  
      • Certified true copy of PSA birth certificate 
      • PSA certificate of no marriage
