For Existing Partners and Dependents
Your existing Avega ID Card is still valid for use. You may present yours when availing at an accredited facility.
For New Members
You may present Avega’s Certificate of Coverage (COC) along with your company ID and another valid ID. You may download the COC letter from your
www.benefitsmadebetter.com account under the
Downloadable Forms tab.
The COC establishes your and your dependent’s HMO plan coverage in the absence of an HMO card.
For principal members and/or dependents that are confined in the hospital and will need doctor consult, a COC needs to be secured and submitted to the hospital admin and/or presented during the consultation.
